Acknowledgment of Risks:

I acknowledge that participating in fitness activities at Fang by Tienji involves inherent risks, including but not limited to physical injury, muscle strains, and other health-related issues. It is my responsibility to consult with a physician before starting any exercise program.

 

Waiver and Release of Liability:

In consideration of being permitted to participate in activities at Fang by Tienji, I, for myself, my heirs, executors, and administrators, hereby release and forever discharge Fang by Tienji, its owners, employees, agents, and representatives from any and all claims, liabilities, or demands of any kind arising from my participation in activities at the facility.

 

Indemnification:

I agree to indemnify and hold harmless Fang by Tienji from any and all claims, damages, or expenses, including attorney fees, arising from my participation in any activities at the fitness center.

 

Medical Consent:

I affirm that I am in good health and have no medical conditions that would prevent my participation in fitness activities. I understand it is my responsibility to inform the center of any medical conditions or changes in my health status.

 

Photographic Release:

I grant Fang by Tienji the right to use photographs or videos taken of me during my participation in activities for promotional purposes.

 

Agreement:

By checking the box on the bookings form, I confirm that I have read, understood, and voluntarily agree to the terms of this Liability Waiver and Release Form.
